POSITION PURPOSE

This position is a medication management pharmacist that serves two primary roles: 1) telephonic engagement of members and their care team for the purpose of improving health and/or providing more cost-effective care and 2) contributes to the strategy, development, implementation, and maintenance of pharmacy population-based clinical strategies.

NATURE AND SCOPE

  • This role does not manage people

  • This role reports to this job: Manager, Pharmacy Population Health

  • Necessary Contacts: To do this job effectively, the incumbent has to be in contact with: all levels of BCBSLA staff including Pharmacy, Care Management, Medical Directors, and Analytics as well as vendors contracted with BCBSLA. This person also communicates with members, their pharmacies and prescribers.

QUALIFICATIONS

Education

  • Doctorate of Pharmacy or Bachelor's Degree in Pharmacy required.

Work Experience

  • 1 year of experience working in clinical pharmacy roles is required.

  • Managed care experience and/or MTM certification is preferred.

  • Other preferred experiences include: • Rounding with physicians and ancillary staff in hospital or primary care settings. • Performing comprehensive medication reviews. • Long-term care consulting experience • Specialty medication management.

Skills and Abilities

  • Must have proficiency in pharmacology, therapeutics, and knowledge of medication management programs for the purpose of supporting the clinical aspects of this job.

  • Must possess strong interpersonal, organizational, oral/written communication skills in addition to strong clinical skills.

  • Ability to use Word and Excel is required; Proficiency in PowerPoint is preferred.

  • Familiarity with Epic Healthy Planet is preferred.

Licenses and Certifications

  • Current pharmacy license in good standing required.

  • Participation in ASHP or AMCP-accredited residency program and/or master's training preferred.

ACCOUNTABILITIES AND ESSENTIAL FUNCTIONS

  • Efficiently assesses medication therapy for unnecessary medications, need for additional medications, member understanding, ineffective medications, inappropriate doses, adverse drug events, drug interactions, cost savings opportunities, and opportunities for medication synchronization.

  • Delivers telephonic medication management services to eligible members to assess appropriateness of all medications, identify and resolve barriers to adherence, optimize medication therapy, address medication concerns, and improve clinical outcomes.

  • Supports the development, implementation, monitoring, and maintenance of pharmacy population-based clinical programs with the purpose of identifying and solving medication related problems, improving patient outcomes, and lowering cost to achieve the organization’s strategic goals.

  • Develops and utilizes external and internal stakeholder relationships to enhance member experience, improve clinical outcomes, and lower costs.

  • Accountable for complying with all laws, regulations and accreditation standards associated with duties and responsibilities.
